Patient Forms
Please fill out one "Dental and Medical History" form per child. You only need to fill out the "Financial & Office Policies" and "Notice of Privacy Practices" forms once for the whole family. You may also fill these out electronically at the first visit, but please arrive 5-10 minutes early.
